Scottish. In use in west Scotland form 17th century. Possibly from Old German "ivo" or "iv", meaning "yew". It has been out of fashion as a male name for a very long time.  Chambers suggests it is a Scottish pet name for ivor although the latter is infrequent in Scotland.
